It’s up to all of us to keep all of us safe!
Fixing a problem on the spot should be a quick, easy solution and something that you are totally familiar with doing – for example, like mopping up a water spill on the floor, or sweeping up a glass tumbler that has been dropped and has shattered.
For everything else, there is a process in place. Reporting an incident, near miss or hazard in the workplace is a critical way for Uniting AgeWell to:
gather information about safety; and
put in place strategies to either stop a similar incident occurring or minimise the impact of a similar incident.
Incidents, near misses and hazards
An incident is an event or circumstance that could have or did result in unintended harm to someone or to property. Incidents can involve residents, clients, staff, volunteer, contractor, students, relatives, or visitors.
Incidents can also be a near miss which incorporates those unplanned events that had the potential to cause harm, but did not result in injury or damage due to timely intervention and/or luck.
A hazard is a situation that has the potential to cause injury, illness, or damage to property.
After lodging an incident form in RiskMan your report will be automatically escalated to the appropriate manager for investigation. Your manager will discuss the outcome of the incident with you.
Use RiskMan to record incidents, near misses or hazards
RiskMan is the system used to capture all workplace incidents, near misses or hazards.
All staff can access RiskMan and should any of these occur, lodge the incident and also notify your manager. It is best to complete the incident form on the same day so it is still fresh in your mind. Early reporting of an incident also means your manager can start the investigation immediately and corrective measures can be implemented quickly.
